On this basis, it is our strict company policy that all requests for service calls MUST be submitted on the below Service Request Form. Please complete all areas of the form and return to email@example.com if you wish to lodge a service.
Please note that you will only be charged if the cause of the issue is something that is NOT covered in the Lifestyle Windows Warranty. Unfortunately we are unable to completely determine this until we have one of our servicemen attend to the site.
We also encourage you to send us through photographs of the problem to help us better diagnose the issue and try to determine a solution before attending site.
